PSY 4949 Liability Insurance Waiver
- Liability Insurance Waiver Memo — if you are volunteering with an approved PSY 4949 agency that does not provide liability insurance (it will say so specifically in the agency’s description in the Community Volunteer Opportunities spreadsheet), you must complete this memo along with your PSY 4949 registration form.
